Responsibilities:
The Licensed Physical Therapist provides coordinated care to patients of all age groups.
Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
Plans, organizes, and conducts physical therapy treatment program based on the medical referral and their evaluation.
Participates in the coordination of care.
Performs ongoing clinical assessment, including OASIS assessments at appropriate time points to identify patient needs and determine level of functioning and appropriateness of patient for home care Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s) Coordinates care and documents coordination with all physicians and other health care clinicians involved in the care of the patient as needed and appropriate Provides patient caregivers and family education Documents, prepares, and completes clinical notes, verbal orders and other clinical record documentation on an ongoing basis and in a timely manner.
Revises the POC based on evaluations and ongoing assessment data Provides treatment and services according to state and federal regulations and professional standards Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease Provides supervision to Physical Therapy Assistants and Aides, when applicable, in the perform of patient care that is consistent with patient needs Meets mandatory continuing education requirement of the Agency and licensing board Uses effective interpersonal relations and communication skills Provides patient caregiver, and family counseling; provides patient and caregiver education Assists and instructs patients and their families in transfer techniques appropriate to patient abilities.
Instructs on use of equipment such as walkers, wheelchair, canes, etc.

Utilizes appropriate therapy equipment specific to patients diagnoses and needs Promotes the Agency philosophy and administrative policies Demonstrates commitment, professional growth, and competency Participates in the Agencys QAPI program Participates in Agency-sponsored in-service training
Qualifications:
Education:
Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved programLicensure:
Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC, to practice Physical Therapy, or if licensure does not apply, meets one of the requirements listed in 42 CFR 484.

Experience:
Prefer one year experience as a Physical Therapist in an acute care, rehabilitation setting or home health.
Skills:
Therapy skills as defined as generally accepted standards of practice Good interpersonal skills Proof of current CPR Knowledge of durable medical equipment Transportation:
Reliable transportation Current Drivers License Valid and current auto liability insurance Recommended Skills Administration Assessments Cpr Clinical Works Communication Health Care Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
